The IQAC has prepared two Google Forms as follows:
1. Incident Form for Students – Clinical Appointments
2. Student Feedback Form – Clinical Appointments
These forms have been introduced to document any incidents encountered during clinical appointments and to obtain your feedback on the overall clinical experience. The information collected will assist in monitoring, addressing concerns where necessary, and implementing measures to further improve the quality of clinical training.
1. Incident form for students for clinical appointments- This form is intended to report any adverse events or incidents encountered during your clinical appointments. The information provided will be treated confidentially and used solely for improving clinical training and ensuring student safety.
2. Student feedback on clinical appointments- This form is to obtain your feedback regarding the clinical appointments you have completed. Completion of this form is not mandatory following each appointment. It may be submitted as required, particularly in response to specific needs of students or suggestions for improvement.
The Internal Quality Assurance Cell (IQAC) will monitor and review the submitted responses at regular intervals. In instances where feedback or incidents are reported, the relevant Clinical Coordinator and the respective Unit will be formally notified for appropriate action. The IQAC will subsequently follow up to ensure that corrective or preventive measures have been duly implemented and documented in accordance with established procedures.
Please complete and submit the relevant form in accordance with the given instructions. Your responses will be valuable in supporting the continuous improvement of clinical appointments.
